Occupational Therapist Supervisor, Float- All Boroughs
New York City (All Boroughs), Westchester, and Long Island
This is a Full-time position which services all five boroughs of New York City, as well as Westchester County and Long Island. The ideal candidate must be flexible and willing to travel to all assigned locations.
Pay Rate: $120,000.00-$130,000.00/ annually

Job Summary: As a Full-Time, Salaried Occupational Therapist Supervisor with Personal Touch Home Care, you will provide both clinical and supervisory oversight for occupational therapy services delivered in patients’ homes across multiple locations, including all five boroughs of NYC, Westchester, and Long Island. This role includes assessing patient needs, developing and implementing treatment plans, and supervising occupational therapy staff to ensure high-quality, compliant, and patient-centered care. The position focuses on improving patients’ quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes.
Key Responsibilities:
1. Conduct assessments of referred patients requiring occupational therapy and rehabilitation services.
2. Provide direct patient care, including evaluating medical conditions, functional capabilities, limitations and restrictions and rehabilitation potential.
3. Establish and administer individualized treatment programs with specific, measurable goals in collaboration with the Physician.
4. Set realistic, achievable goals based on patient capacity and tolerance.
5. Complete and maintain accurate clinical documentation, including evaluations, weekly progress notes, recertifications, and discharge summaries.
6. Educate patients and families on appropriate therapeutic techniques and home programs.
7. Collaborate with interdisciplinary team members to provide the best patient centered treatment and care.
8. Provide clinical supervision, guidance, and support to Occupational Therapists and Occupational Therapy Assistants, as applicable.
9. Review documentation and treatment plans to ensure compliance with professional standards, company policies, and regulatory requirements.
10. Assist the Director of Patient Services/ Rehabilitation with staff training, mentoring, performance feedback, and quality improvement initiatives.
11. Support scheduling, caseload management, and clinical problem-solving as needed.
12. Perform other duties as assigned by the Director of Patient Services/ Rehabilitation.
13. Ensure compliance with confidentiality standards, HIPAA regulations, and Federal Residents’ Rights.
Qualifications:
Must have graduated from an accredited Occupational Therapist college or university.
Must have a valid professional license and registration for state of practice.
Experience with long-term care and/or geriatric care is preferred but not limited to.
Demonstrates good verbal and written communication and organization skills.
Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.
