Pay:
LPN: $35–$40/hour (DOE)
RN: $38–$43/hour (DOE)

Job Summary:

Aspen Meadows Health and Rehabilitation Center is seeking a hybrid MDS Coordinator / Social Services professional to support both clinical reimbursement and resident psychosocial needs. This unique dual-role position partners closely with the Interdisciplinary Team (IDT) to ensure accurate MDS completion, effective care planning, and strong discharge coordination for residents.

This role is ideal for a nurse who enjoys both the analytical side of MDS and the relationship-driven work of care coordination and social services.

Essential Functions:

MDS / Clinical Reimbursement Duties:

  • Coordinate and complete MDS, CAAs, and care plans in accordance with State and Federal regulations

  • Set and communicate Assessment Reference Dates (ARDs) with the IDT

  • Ensure timely and accurate completion of all MDS assessments

  • Review clinical documentation to support MDS coding and reimbursement

  • Participate in Medicare and insurance utilization management

  • Collaborate with nursing and therapy teams to ensure compliance and accuracy

Social Services / Case Management Duties:

  • Support residents and families with psychosocial assessments and care planning

  • Assist with discharge planning, transitions of care, and follow-up services

  • Connect residents and families with community resources (financial, legal, mental health, etc.)

  • Provide support for residents experiencing behavioral or psychosocial challenges

  • Partner with IDT to develop and implement person-centered care plans

  • Act as a liaison between residents, families, and the care team

Education, Licensure, and Experience:

  • Required: Active RN or LPN license in the state of employment

  • Preferred: Experience with MDS/RAI process and care planning

  • Preferred: Long-term care or skilled nursing experience

  • Preferred: Experience with discharge planning, case management, or social services

  • Preferred: PointClickCare (PCC) experience

  • Strong communication, organization, and interdisciplinary collaboration skills required
