Job Title: Family Nurse Practitioner (FNP)
Reports to: Family Medical Doctor / Medical Director

Job Summary
Delivers high-quality primary and preventive care for adults in an integrated community-mental-health setting, focusing on chronic-disease management, wellness promotion, and collaboration with behavioral-health teams.
Key Responsibilities
-Conduct full health assessments, diagnose & manage acute/chronic conditions.
-Provide medication management, patient & family education, and minor procedures.
-Order/interpret diagnostic testing.
-Collaborate with physicians, therapists, case managers; mentor nursing staff.
-Maintain thorough EHR documentation and participate in quality-improvement initiatives.
Qualifications
-MSN or DNP, national NP board certification (AANP/ANCC).
-Active IL NP license; Controlled Substance/DEA & Full-Practice Authority preferred.
-≥1-2 yrs primary-care NP experience; behavioral-health exposure valued.
-Proficiency with EHR (athenaOne) and strong teamwork/communication skills.
-Work Environment & Physical Requirements
-Ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
