GENERAL FUNCTION:  

Responsible for providing case management services to youth and young adults experiencing clinical high risk for psychosis (CHRP) and first episode psychosis (FEP) with the goal of improving client functioning and promoting wellness across multiple domains.

 

SUPERVISORY RESPOSIBILITY:  None                                                      

 

QUALIFICATIONS, KNOWLEDGE AND REQUIRED TRAINING:  

  • Bachelor’s degree in social or behavioral science or related field.
  • Must have or be willing to obtain CPR certification.
  • Must possess a valid driver’s license and proof of auto insurance, and a willingness to transport clients in personal vehicle.
  • Must have good communication skills and a natural ability to establish rapport with youth/young adults and families, as well as work collaboratively as a member of a team.
  • Excellent organizational and communication skills, both oral and written.

 

AMOUNT OF TRAVEL: Local and statewide travel as needed (Approximately 50-60%)

 

DUTIES AND RESPONSIBILITIES:

  • Provide office, home, and community-based care coordination services for youth/young adults and their families, increasing or reducing intensity based on need.
  • Attend local, statewide, and national trainings and seminars as appropriate.
  • Models and teaches effective advocacy skills, provides caregiver active support.
  • Ensure coordination, referral and linkage of services to assigned consumers, including referrals for counseling/therapy, A&D treatment and other services needed.
  • Ensure individuals are notified of upcoming appointments.
  • Collaborate with other team members to address client issues and work toward treatment goals.
  • Maintain documentation, records, and reports as established by the agency, SAMHSA, TDMHSAS, etc.
  • Participate in on-call rotation and be available 24/7 during their on-call week (approximately every 6th week) to program clients who may be experiencing a mental health crisis.
  • Other duties as assigned.

 

PERSONAL RELATIONSHIPS:  In order to accomplish their duties, the person occupying this position will be required to maintain effective relationships with the professional clinical staff and the clients of Alliance Healthcare Services.
