The Viral Hepatitis & TB Care Coordinator plays an important role in supporting NEMS’ Viral Hepatitis B/C and Tuberculosis (TB) Programs through patient care coordination and outreach. This role is responsible for supporting patient engagement and retention strategies that contribute to improved viral hepatitis, TB, and other health-related outcomes. The Care Coordinator will connect current and prospective patients with NEMS viral hepatitis and TB services, promote preventative care, and increase access to culturally and linguistically appropriate healthcare resources.
The candidate must demonstrate strong communication and relationship-building skills to work effectively with patients and families, healthcare providers and care teams, and community-based organizations. These skills will support improved care coordination, help identify and address cultural and healthcare system barriers, promote timely access to care, and empower patients to engage in preventative services and population health programs.
ESSENTIAL JOB FUNCTIONS:
- Conducts targeted telephone and/or face-to-face outreach and education for hard-to-reach and high-risk communities.
- Provides education on viral hepatitis B/C, TB, other communicable disease prevention, testing, vaccination, and support services.
- Schedules primary care, radiology, vaccine, and other appointments for patients.
- Places standing orders for viral hepatitis B/C and TB screening tests, liver cancer surveillance ultrasounds, and other diagnostic tests.
- Identifies and understands the needs, interests, and barriers affecting current and prospective patient populations to increase awareness and utilization of services offered by NEMS.
- Motivates and advocates for patients to be active and engaged in their health.
- Practices cultural humility and adopts a trauma-informed approach to increase confidence in vaccinations and treatment options.
- Conducts data collection, tracking, and reporting of outreach activities and outcomes related to grant programs, research, and other initiatives
- Maintains and validates reports to meet project and performance reporting requirements.
- Coordinates patient care and outreach activities for grant programs and research with local/regional health departments, academic and community partners, and other stakeholders to ensure program goals are achieved.
