Position Summary

The Intake Coordinator is responsible for processing referrals, verifying insurance, and coordinating admissions in a fast-paced home health environment. This role works closely with scheduling staff and the RN Admission Manager to ensure timely and accurate patient onboarding.

Responsibilities

  Process and enter all incoming referrals accurately and efficiently

  Verify insurance coverage and document confirmations

  
  


• 
  

  Triage service orders and Start of Care documentation

  
  


• 
  

  Obtain signed physician orders and required clinical documents

  
  


• 
  

  Research and secure missing referral information

  
  


• 
  

  Maintain accurate electronic records and referral logs

  
  


• 
  

  Perform additional duties as assigned
