Erie Family Health Centers, a top workplace in Chicago and the suburbs, is looking for a valuable addition to our Patient Program and Support Services! The Population Health Navigator role is a patient-facing role that helps improve the agency’s quality of health care through targeted, data-driven outreach. Outreach is based on agency priorities, including chronic disease and preventative health needs and specialty care referral navigation, and includes a variety of methods such as campaign texting, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others. The team also assists patients with any health-related social needs that are presented as barriers to care.

Main Duties & Responsibilities
- Screen Clients for needs around social drivers of health and connect to internal and external resources. Take on short-term agency initiatives related to population health outreach, as they arise.
- Link to and/or enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.).
- Assist patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health.
- Conduct health outreach activities, through a variety of methods including campaign texting, appointment recalls, 1on1 patient phone calls, bulk referral ordering, and MyChart, as well as others.
- Support patients with scheduling specialty referrals with outside hospital partners.
- Provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.
