Position Overview

Avail Health & Behavioral Solutions is seeking a reliable, organized, and compassionate Supported Living Coach (SLC) to provide skill-building and community-based support services to individuals with developmental disabilities.

The Supported Living Coach assists individuals in maintaining independence in their home and community settings while working toward goals outlined in their Person-Centered Support Plan.

This position requires strong documentation skills, professionalism, and the ability to work independently in the field.

Key Responsibilities

  • Provide individualized support in home and community settings

  • Assist individuals with budgeting, meal planning, and household management

  • Support development of independent living skills

  • Reinforce behavioral strategies as outlined in support plans

  • Coordinate with families, support coordinators, and interdisciplinary teams

  • Transport clients as needed (appointments, community activities)

  • Maintain clean, accurate, and timely documentation

  • Complete required reports and submit case notes per agency policy

  • Promote dignity, safety, and client rights at all times

Qualifications

  • High school diploma or GED required

  • Experience working with individuals with developmental disabilities preferred

  • Knowledge of APD standards and person-centered planning preferred

  • Strong written and verbal communication skills

  • Ability to work independently with minimal supervision

  • Valid driver’s license and reliable transportation

  • Must pass Level II background screening
