Job Title: Field Nurse (RN) – Home Care

Job Summary:
The Field Nurse is responsible for conducting in-home patient assessments, completing reassessments, and performing supervisory visits to ensure quality of care in accordance with agency policies and state regulations. The Field Nurse works under the direct supervision of the Director of Nursing (DON) and plays a key role in coordinating patient care, maintaining clinical standards, and supporting field staff.
Key Responsibilities:
• Perform initial and ongoing nursing assessments for new and existing patients in their homes.
• Complete timely reassessments in accordance with state and agency guidelines.
• Conduct supervisory visits to monitor and evaluate caregiver performance and patient satisfaction.
• Develop, update, and implement individualized care plans based on patient assessments.
• Report changes in patient condition to the DON and assist with care plan modifications as needed.
• Educate caregivers, patients, and families on clinical care, medications, and home safety as appropriate.
• Document all visits, assessments, and communications accurately and timely in the electronic health record (EHR) system.
• Maintain compliance with state regulations, agency policies, and best clinical practices.
• Participate in case conferences and communicate regularly with the clinical team to ensure coordinated care.
• Support the agency’s quality assurance and performance improvement initiatives.
• Maintain professional licensure and complete all required in-service education.
Qualifications:
• Current, active RN license in the State of New Jersey (Required).
• Minimum of 1 year clinical nursing experience (Home care experience preferred).
• Strong assessment, communication, and organizational skills.
• Ability to work independently in a field setting with minimal supervision.
• Valid driver’s license, reliable vehicle, and willingness to travel within assigned territory.
• Proficiency with electronic documentation and basic computer skills.
