ABOUT THE ROLE:

The MAT care manager cares for patients accessing MAT services at Community Health and also coordinates with their care team members and other supports to help a patient manage their substance use, medical conditions and co-occurring behavioral health, psychological and social determinants of health through the healthcare system. MAT Care managers work closely with the primary care providers to establish treatment plan goals.
      

FUNCTIONS OF THE POSITION: 

  • Provide direct nursing and care management services to MAT patients.
  • Act as liaison between MAT provider and medical provider to ensure that patient is up to date on medical needs.
  • Provides support and coordination to MAT patients in preventing barriers to care and promotion of community resources and healthcare services.
  • Provide follow-up phone calls and any other services post discharge to MAT patients discharged from an inpatient, outpatient, or residential program.
  • Conduct MAT intakes and present potential new patients to MAT team.
  • Work collaboratively with patients in psychiatric and substance abuse programs.
     

SKILLS REQUIRED FOR SUCCESS:

  • Current Vermont RN license, LPN may be considered.
  • CPR Certification.
  • Prior experience working in a nursing position required; prior case management experience in a similar outpatient setting preferred.
  • Experience in using a variety of electronic medical record and ability to learn other systems, basic keyboarding skills and email communication.
