Job Title: Family Preservation Social Worker
Location: Hybrid – Wellspring Bay City
Service Area: Saginaw, Bay, Midland, Clare and Isabella counties.
Reports to: Family Services FTBS Supervisor – Bay City

Wellspring is a statewide nonprofit serving seniors, families and children across Michigan. We are currently seeking a Family Preservation Worker to join our Families Together Building Solutions (FTBS) Program in Bay City. In this role, you will directly support families across Mid-Michigan.
Responsibilities
- Adhere to the Families Together Building Solutions (FTBS) model using a strength-based, non-deficit approach.
- Assist families in accessing concrete services including housing support, transportation, home cleaning, maintenance and repairs.
- Provide skills training in parenting, communication, self-advocacy and self-sufficiency.
- Offer short-term counseling and crisis intervention for families experiencing stress or instability.
- Coordinate collateral contacts for outpatient counseling, medical care, educational needs, utility assistance and housing resources.
- Prepare and maintain required documentation including service plans, progress reports, termination summaries, goal sheets, weekly plans, time reports and correspondence within required timeframes.
- Present cases at termination conferences and recommend appropriate follow-up services.
- Maintain 24-hour availability to clients and a flexible work schedule based on family needs.
- Conduct face-to-face contact according to contract requirements and individual family needs.
- Maintain regular communication with referring workers.
- Complete follow-up evaluations at 3, 6 and 12 months within designated timeframes.
- Transport clients using your own vehicle as needed.
Qualifications
- Bachelor’s degree in social work or a related human services field required.
- Current and valid driver’s license, vehicle registration and auto insurance required.
- Experience working with children and families preferred.
- Reliable, responsible and compassionate approach to family support.
- Strong organizational, communication and documentation skills.
Schedule and Compensation
- Employment Type: Full-Time
- Schedule: Flexible schedule with 24-hour on-call availability as needed.
- Hybrid-Remote: Home-based with travel throughout Saginaw, Bay, Midland, Clare and Isabella counties.
- In-Office Requirements: Weekly team meetings, trainings and clinical supervision.
- Salary: $43,000 per year, based on experience.
- Mileage Reimbursement for work-related travel.
