Job Description

Aventura at the Bay is looking for a qualified Director of Social Services to join our amazing leadership team!

We are seeking a compassionate and experienced Social Services Director to lead and coordinate the social services department within our skilled nursing facility. This role is responsible for ensuring the emotional, psychosocial, and discharge planning needs of our residents are met while maintaining compliance with all federal and state regulations.

Purpose

As the director, you are responsible for providing services to respond to the emotional needs of the residents and their families. You will also assist with resident admissions and the referral process.

Minimum Qualifications

  • Bachelor’s Degree in Social Work or related field required (Master’s preferred)
  • 1 year of Social Worker experience required
  • Prior experience in a skilled nursing or long-term care setting preferred
  • Strong knowledge of discharge planning, care coordination, and regulatory compliance
  • Excellent communication, organizational, and interpersonal skills
  • Compassionate, resident-centered approach to care

Key Responsibilities

  • Assess residents’ psychosocial needs upon admission and throughout their stay
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Coordinate discharge planning to ensure safe and appropriate transitions of care
  • Provide counseling and emotional support to residents and families
  • Act as a liaison between residents, families, staff, and community resources
  • Ensure compliance with all regulatory standards and documentation requirements
  • Advocate for resident rights and promote quality of life initiatives
  • Supervise social services, staff
