Job Title:  Certified Nursing Assistant (CNA)

Supervisor:  Adult Day Health Coordinator

Positions Supervised:  none

Pay Range:  To be determined

FLSA Status:  Non-exempt Full-Time

 

Job Summary   

Assists participants with activities of daily living; Assists with other activities and tasks related to participant care and Center operations under the supervision of a Registered Nurse (RN); Maintains a clean and safe environment.

Specifications   

Education and Special Training:  High school degree, or equivalent; Currently listed as a Nursing Assistant (NA) I with the North Carolina Department of Health and Human Services (NC DHHS) with no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home; Experience:  At least 1 year experience with aging, frail elderly; 6 months clinical experience as a Nursing Technician, Nursing Assistant, Corpsman or Emergency Medical Technician.

Licensure, Registry or Certification Required:  Certificate from an approved CNA training program; listed as a CNA I with the NC DHHS; Valid NC driver’s license and vehicle.

Knowledge, Skills, and Abilities: Must be able to work effectively in a team environment.  Act only within the scope of his or her authority to practice.  Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid before working independently.  

 

With or without reasonable accommodations:  Ability to bend and or stoop frequently; Have independent mobility in order to climb stairs and walk distances; Lift and position patients of all weights and heights utilizing correct body mechanics; Lift and carry pieces of durable medical equipment; Lift and transport at least 50 pounds during the course of providing care, treatment and/ or services; Utilize correct body mechanics to transfer, lift, and position participants; Read, write, and speak in English; Communicate effectively both orally and in writing; Ability to clearly see medical records and various documents; Communicate via telephone; Hear blood pressures and spoken communication; Ability to perform repetitive hand/wrist movements.  

Summary

Key Responsibilities   

       Provides basic personal care assistance and assistance with other activities of daily living to assigned participants.

       Provides assistance with nursing care, restorative therapies, activities and meals across care settings.

       Documents observations, treatments, procedures, nursing care and participant activities appropriately.

       Works with other members of the Interdisciplinary Team (IDT) on coordination of 24 hour care delivery.

       Communicates effectively with participants, families, staff, and other departments and agencies.

       Works effectively in a team setting.

       Abides by all established policies, rules and regulations.

       Participants as a member of the IDT and takes part in participant care planning.  Communicates any changes in participant condition to other team members.

       Attends staff meetings.

       Other duties as assigned. 

 

Environmental Factors  

Tasks with potential for exposure to blood borne pathogens include, but are not limited to the following:

1.      Assisting with mouth care

2.      Assisting with care of non-intact skin

3.      Assisting incontinent participants with personal care

4.      Administering an enema

5.      Shaving participants

6.      Assisting with procedures in the clinic

 

Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
