Physical Therapist

Job Summary:

Pearl Pointe, a leading provider of Skilled Nursing services in Freeport, Illinois, is seeking a compassionate and skilled Physical Therapist to join our dedicated team of healthcare professionals. As a Physical Therapist at Pearl Pointe, you will have the opportunity to make a meaningful difference in the lives of our patients by providing personalized physical therapy services designed to meet their unique needs and goals.

Responsibilities:

• Develop and implement individualized physical therapy plans to improve mobility, function, and overall well-being of patients

• Conduct thorough assessments and evaluations to identify physical therapy needs and goals

• Provide one-on-one and group therapy sessions to patients, including exercises, stretches, and other interventions

• Collaborate with other healthcare professionals, including nurses, occupational therapists, and speech therapists, to ensure comprehensive care

• Document patient progress and treatment plans, maintaining accurate and timely records

• Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement

Requirements:

• Current physical therapy license in the State of Illinois

• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals

• Ability to work in a fast-paced, dynamic environment with a focus on delivering high-quality patient care
