JOB SUMMARY
The Case Manager serves as the primary coordinator of services for participants enrolled in the PWDC program. This role assists participants in navigating healthcare systems, accessing behavioral health treatment, and connecting to social services such as housing, childcare, and employment support.
The Case Manager works closely with clinicians, peer recovery specialists, and other program staff to ensure participants receive coordinated care that promotes recovery, maternal wellness, and healthy family outcomes.
JOB DUTIES
Conduct intake assessments to identify participant needs and service goals.
• Develop individualized service plans for program participants.
• Coordinate referrals to healthcare providers, behavioral health services, and community resources.
• Monitor participant progress and maintain regular communication with participants.
• Assist participants with accessing resources such as housing, transportation, and childcare.
• Collaborate with multidisciplinary team members to coordinate services.
• Maintain accurate documentation in the electronic health record system.
• Participate in team meetings and case reviews.
QUALIFICATIONS
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
Experience in case management, social services, or behavioral health preferred
Experience working with vulnerable populations including pregnant women or families preferred
Valid driver’s license and reliable transportation required for community-based services and home visits.

Maintain 90% compliance with documentation requirements
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
Ability to sit for extended periods during documentation and meetings
Ability to travel within the community for home visits and meetings
Ability to lift up to 15 pounds occasionally
- Services are provided in both office and community settings, including participant homes when appropriate.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
