Position Summary:
We are seeking a Wound Care focused Physician Assistant (PA-C) to provide on-site rounding, advanced wound management, and comprehensive geriatric care. In this role, you will manage complex clinical cases, lead treatment planning, and collaborate closely with interdisciplinary teams. This is an opportunity to practice at the top of your license while expanding your expertise in wound care, post-acute medicine, and senior population health.
Responsibilities:
• Provide on-site rounding, primary care, and advanced wound care services.
• Manage complex wounds including pressure injuries, surgical wounds, and diabetic ulcers.
• Drive chronic disease management and preventative care strategies.
• Collaborate with physicians, nurses, therapists, and facility staff.
• Reduce hospital readmissions through proactive clinical management.
• Communicate with families, caregivers, and interdisciplinary teams.
• Complete documentation, medication management, and lab coordination.
Qualifications:
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in wound care, geriatrics, primary care, or post-acute settings preferred.
• Strong clinical judgment and ability to manage complex cases independently.
• Valid driver’s license and reliable transportation.
• Passion for delivering outcomes-driven, patient-centered senior care.
