LPN Position Summary
As a Licensed Practical Nurse (LPN), you will play a vital role in supporting the health, safety, and dignity of our residents in an assisted living environment. You will provide hands-on nursing care, coordinate treatments, and collaborate with caregivers and leadership to ensure high-quality resident outcomes.
Key responsibilities include:
Resident assessments and care planning
Medication administration and treatment management
Monitoring and documenting changes in resident condition
Accurate clinical documentation
Supporting resident independence while maintaining dignity
Collaborating with caregivers, families, and healthcare partners
Qualifications & Requirements
Active State of Connecticut LPN License (required)
Prior experience in assisted living, senior living, long-term care, or skilled nursing preferred
Strong clinical judgment and attention to detail
Compassionate, resident-centered approach to care
Ability to work effectively as part of a team
