You’ll be a guiding hand, a trusted support, and an essential part of our caregiving community. From assisting with therapeutic exercises to promoting mobility and independence, your daily work will have a lasting and meaningful impact.

Key Responsibilities

  • Assist residents with mobility training, gait training, transfers, and strengthening exercises

  • Implement individualized therapy treatment plans according to resident needs

  • Monitor resident progress and document outcomes accurately and timely

  • Provide education and support to residents and their families regarding therapeutic activities and home exercise programs

  • Maintain a safe and clean therapy environment, ensuring equipment is used appropriately

  • Collaborate with nursing staff, therapy teams, and interdisciplinary teams to support overall care plans

  • Encourage resident participation and promote independence in daily functional activities

Qualifications for a Skilled Nursing Facility

  • Completion of an accredited Physical Therapist Assistant program

  • Strong communication and interpersonal skills, especially with elderly or rehabilitating residents

  • Ability to work collaboratively with Physical Therapists, nursing staff, and other care team members

  • Ability to lift, support, and assist residents safely during mobility and therapeutic activities
