SOUTHWESTERN PA AREA AGENCY ON AGING



 



Overall Function:



 



To provide care management services to consumers receiving services through the Caregiver Support and Domiciliary Care Programs at the Southwestern PA Area Agency on Aging.



 



Essential Functions:



 




  • Complete comprehensive psychosocial assessments with potential consumers and families to determine program eligibility (both programs) and placement needs (Dom Care only).

  • Develops and maintains an individualized service plan (ISP) required for consumer, and oversees that services are being utilized.

  • Conducts follow-up visits at prescribed intervals, maintaining regular contact with consumer and provider to ensure mutual satisfaction within program guidelines.

  • Provides assistance and counseling to consumers, providers and families ensuring service delivery and social and emotional support and identifying other community resources that consumers may be able to utilize.

  • Maintains a current consumer case record, consumer assessments, placement processing (Dom Care only), referrals, updates, contacts, follow-up visits, any problems and the course of action pursued.   

  • Prepares and submits all required case forms, reports, financial paperwork/documentation, and correspondence in an accurate and timely manner in accordance with program regulations.

  • Initiates and maintains contact with appropriate health care providers and human services programs to ensure the protection of the program consumer's rights and benefits.

  • Facilitate or assist with monthly consumer support groups, education, recognition events, etc.

  • Participates in case conferences, meetings, and trainings as scheduled.

  • Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.


Qualifications



  • Bachelor’s Degree in Social Work or related field.

  • Demonstrated knowledge of financial entitlement programs and health/social services available to the consumers.

  • Demonstrated ability to make incisive observations and obtain vital information during the client/family interview and/or while reviewing case records to make appropriate recommendations.

  • Must not be excluded from participation in Medicare, Medicaid, or any other federal health care programs.

  • Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearance.

  • Ability to operate a variety of agency vehicles or transport oneself, as necessary, in the performance of essential functions.

  • Ability to maintain consumer caseload.

  • Ability to assist, move, and/or transport durable medical equipment, medical supplies and/or consumer's personal effects weighing up to 50 pounds.

  • Ability to pack and transport cognitively and/or physically challenged consumers as needed.
