Job Summary
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the hospice plan of care, providing treatments, and working collaboratively with the interdisciplinary team to support comfort-focused patient care.
Duties & Responsibilities
Provides direct patient care as defined in the State’s Nurse Practice Act
Implements plan of care initiated by the registered nurse
Provides accurate and timely documentation consistent with the plan of care
Collects assessment data and reports observations to the RN and physician as appropriate
Assists in coordination of hospice services with interdisciplinary team members
Provides patient and caregiver education and emotional support
Uses equipment and supplies effectively and efficiently
Participates in professional growth and development
Performs other duties as assigned by the registered nurse

Job Requirements
(Education, Experience, Knowledge, Skills & Abilities)
Completion of an accredited practical (vocational) nursing program
Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
Three years nursing experience preferred
Hospice, home health, or medical-surgical experience preferred
Complies with accepted professional standards and practice
Demonstrates strong verbal and written communication skills
Strong organizational skills
Must be a licensed driver with an insured and reliable automobile
Maintains current CPR certification
