JOB SUMMARY:

The Job Coach provides training and support to individuals with disabilities to maintain and enhance integrated community employment. Serve as a liaison between the individual and the employer to facilitate the necessary supports for the individual to become a successful employee.

ESSENTIAL FUNCTIONS: 

  1. Provide job skills training and on-going supports to include completion of assigned job tasks, appropriate workplace social skills, problem solving, job adaptations, and other job- related skills.
  2. Analyze job duties and break down into manageable tasks to facilitate learning.
  3. Maintain ongoing communication with the employer, as needed, to address problems, concerns, or needs that may arise.
  4. Monitor quality of individual’s work to ensure it adheres to company’s standards and all tasks are completed within acceptable time frames.
  5. Facilitate development of natural supports for individuals receiving services to build and maintain positive working relationships with co-workers, supervisor, and customers.
  6. Complete daily documentation of progress notes and other reports in an accurate, timely and professional manner.
  7. Provide input into the development of the individual person-centered plan.

 

QUALIFICATIONS:

  • Must be willing to work after-hours and/or on weekends, as required.
  • Excellent interpersonal and communication skills
  • Ability to project a professional image while interfacing with the community
  • Adaptable/flexible – enjoy doing work that requires frequent shifts in direction
  • Basic computer skills to include keyboarding
  • Must be able to work independently with limited supervision AND must be able to work in a team environment and produce results in conjunction with fellow team members. 






EXPERIENCE/EDUCATION REQUIRED:

One-year successful experience working with persons with various disabilities to include developmental disabilities, mental health issues, and substance use disorders desired

High school diploma or equivalent required

Valid driver’s license and safe driving record
