ABOUT THE TEAM
The Community Engagement Team assists individuals in their educational, recreational, fitness, and social endeavors with individualized support. They work with each person to create a monthly schedule of well-rounded, meaningful experiences that provide personal enrichment and prosperous employment. People receiving supports spend their entire day in the community and are involved in activities such as working, volunteering, or participating in job-readiness activities.
IN THIS ROLE, YOU'LL:
- Be the first point of contact and screen communications with family members and community living/residential providers, and resource coordination. Act as front-line resource for staff, individuals and families with person-centered planning, programmatic questions, concerns, and comments. Connect department constituents with mentoring resources and supports.
- Develop and participate in operational systems improvements/innovations within department and across agency.
- Support Community Engagement structural and educational initiatives by creating materials and offering trainings in coordination with Director.
- Follow pre-planning process for PCPs, complete Detailed Service Authorization Tool (DSAT), Review Service Implementation Plans (SIPs), approve services in the Maryland LTSS funding system and implement the final PCP.
- Monitor and enter correct services in ICM data base.
- Develop and/or monitor CE systems for reporting of metrics, service calendars, rosters, daily documentation.
- Assist Program Director and Front- Line Supervisors in recruitment and onboarding efforts for CE. Assist with the scheduling of interviews/shadowing and ensure hiring packet process is complete and timely.
- Liaise with Partnerships Department, Knowledge Management, and external resource organizations, to share information about opportunities, classes, and community resources for staff and people supported in CE.
- Attend weekly CE Management Team meetings and assure minutes taken.
- Attend weekly meetings with Program Director and keep Director abreast of important program matters and changes.
- Assist with special department projects on as needed basis.
ABOUT YOU
- Minimum of 5 years of experience in relevant field
- BA/BS degree
- Knowledge of services for people with disabilities preferred.
- Strong organization, prioritization, and problem-solving skills.
- Strong working knowledge and practice of the Microsoft Office Suite, including comfort with Excel spreadsheets
- Experience with client database and/or billing systems is preferred.
