What You’ll Do:
• Complete Start of Care (SOC) admissions in patients’ homes.
• Conduct detailed clinical assessments and OASIS documentation.

What You Bring:
• Active Minnesota RN license.
• Experience with home health admissions and OASIS preferred.

• Reliable transportation.
