Position Summary
As a Registered Nurse (RN), you will plan, organize, and deliver skilled nursing care in the home setting. You’ll work closely with an interdisciplinary team and play a key role in educating patients and families, promoting independence, and ensuring high‑quality outcomes.
This role is ideal for nurses who value meaningful one‑on‑one patient relationships and enjoy working independently while still being supported by a strong clinical team.
Key Responsibilities
- Develop and initiate individualized plans of care in collaboration with the interdisciplinary team, patient, and caregivers
- Provide skilled nursing interventions as ordered by the physician
- Deliver preventative, rehabilitative, and patient‑centered care
- Educate patients and caregivers on disease processes, medications, and home care techniques
- Document clinical notes accurately and timely
- Communicate effectively with physicians and other healthcare professionals
- Participate in discharge planning to ensure safe transitions
- Support patients and families through counseling and ongoing communication
Requirements
- Active Texas RN license
- Home Health experience preferred (but strong clinical experience considered)
- Current CPR certification
- Valid driver’s license and reliable, insured vehicle
- Strong assessment, communication, and problem‑solving skills
- Ability to work independently and manage time effectively
- Physical ability to lift, position, and transfer patients and equipment
- Ability to perform prolonged standing, walking, bending, kneeling, and reaching
Compensation
$85,000 – $95,000 per year This role follows a per‑visit pay structure, and the listed range reflects a good‑faith estimate for a clinician averaging 40 hours per week.
