Medical Case Manager 

JOB: Full Time Status Reports To: Clinic Manager

FLSA Status: Non-Exempt Amount of Travel Required: Minimum

Grade/Level: Positions Supervised: None

Work Schedule

8 a.m. to 5:30 pm Monday- Thursday. 

8 a.m. to 12:00 pm on Friday. 

However, adjustments in schedule or additional hours may be required to complete duties.


JOB SUMMARY: 

The Clinical Case Manager is part of the care team and under the supervision of the Clinic Manager. The Clinical Case Manager primary purpose will be to work in conjunction with all health services within the health center in order to aid in the development of a comprehensive services delivery system aimed at meeting the needs of the patients. The Clinical Case Manager is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of Care.


JOB DUTIES: 

Include but not limited to:

  1. Handle all telephone encounters for their clinic
  2. Be responsible for all clinic paperwork, refill requests, and notification of lab results. 
  3. Process requests from patients and staff via electronic messages.
  4. Interacting with patients and staff on a daily basis in a courteous and professional manner, addressing questions and direct calls promptly.
  5. Communication with management in regards to any related issues.
  6. Additional duties or responsibilities as deemed necessary.
  7. Participates in monthly clinical staff meetings 
  8. Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  9. Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
  10. Participate in daily huddles and maintain an active role within the care team.
  11. Completion of continual job training processes set forth by Mainline Health Systems, Inc.


QUALIFICATIONS: All listed below are key skills necessary for the position. 

  1. Ability to communicate clearly, takes direction, and work well with a team but also self motivated. 
  2. Positive attitude.
  3. Detail oriented.
  4. Ability to navigate and utilize electronic medical records.
  5. One year of experience in medical field/setting.
  6. Unencumbered Nursing License
  7. The ability to work in a constant state of alertness and in a safe manner.


REPORTING RELATIONSHIPS:

Reports directly to the Clinic Manager. 


 

EVALUATION:

Annual evaluation performed by Clinic Manager with consult of Director of Operations and/or COO.

WORK ENVIROMENT:

An office environment in a controlled atmosphere building. This is an OSHA low –risk position.


Salary

The salary for this position is negotiated between the individual and the Clinic Manager using established salary scale. Bilingual employees receive an additional $0.50 per hour.


STATUS:

This is a FLSA Non-Exempt position.
