LPN – Hybrid Clinical & Care Management

Job Summary:

PreventelHealth is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of clinical experts in Cleveland, OH. As an LPN – Hybrid Clinical & Care Management professional, you will have the opportunity to provide exceptional patient care and exceptional quality in every detail. Responsibilities:

• Communicate effectively with patients, family members, and healthcare professionals to ensure seamless care coordination

• Maintain accurate and detailed records of patient care, treatment plans, and medication administration

• Travel to various facilities in the state of Ohio to provide care and support

Requirements:

• Must have an up-to-date LPN degree in the state of Ohio

• Must be willing to travel to various facilities in the state of Ohio

• Must have Nursing Home experience for at least 1 year (Please do not apply if you do not have this)

• Must be personable, as this is a concierge, client-facing job

• 1 year of minimum experience as an LPN
