The Role
We are seeking a highly organized, proactive, and patient-centered Medical Assistant to support our clinical team with outreach, scheduling, and follow-up.
This is a critical role in ensuring patients feel supported and informed throughout their care journey. You will be the bridge between our patients and providers, helping coordinate care and ensure a seamless patient experience.
Key Responsibilities
Patient Outreach & Engagement
- Conduct outbound calls, texts, and emails to engage new and existing patients
- Support patient onboarding, including intake coordination and documentation collection
- Ensure patients understand next steps and feel supported between visits
Scheduling & Coordination
- Schedule and manage virtual appointments across providers and disciplines
- Optimize calendars to ensure efficient, patient-centered workflows
- Coordinate follow-ups, reschedules, and reminders
Follow-Up & Care Continuity
- Track and execute post-visit follow-ups and care plan check-ins
- Monitor patient tasks, referrals, and next steps to ensure completion
- Escalate clinical concerns to the appropriate team members
Administrative & Clinical Support
- Maintain accurate records in the EMR
- Assist with documentation workflows and patient communications
- Support multidisciplinary coordination across the care team
Qualifications
- 1–3+ years of experience as a Medical Assistant, Care Coordinator, or similar role
- Experience in oncology, primary care, or telehealth strongly preferred
- High school diploma or equivalent required; Medical Assistant certification or relevant clinical experience preferred
- Excellent communication skills—clear, empathetic, and professional
- Strong organizational skills and attention to detail
- Comfortable with technology, including EMRs, scheduling tools, and Google Workspace
- Understanding of HIPAA and patient confidentiality standards
