Nursing Home Social Worker
South Range, WI

$26-$34/hour, based on experience

We are looking for a Nursing Home Social Worker to join our team in South Range. This is a great opportunity for someone who works well with residents and families, stays organized, and can help guide admissions, transitions, and discharge planning.

Responsibilities:

• Support residents and families throughout admission, stay, and discharge

• Help address social service, psychosocial, and family concerns

• Coordinate with hospitals, residents, families, and the interdisciplinary team

• Assist with care planning, documentation, and follow-up

• Advocate for resident needs and help create a positive experience

Qualifications:

• Experience in social work, case management, discharge planning, human services, admissions, or a related healthcare setting preferred

• Nursing home experience is a plus, but not required

• Backgrounds from hospitals, rehab, assisted living, behavioral health, hospice, or community-based services are welcome

• Strong communication and organizational skills required
