Position Overview:
Lead the RAI process with precision, ensuring timely and accurate MDS assessments, CAAs, and interdisciplinary care plans. Bring your expertise to a supportive environment where your organizational skills drive quality outcomes.

Key Responsibilities

• Guide timely completion, input, and state transmission of MDS assessments.

• Facilitate care plan conferences and monthly Quality Measure (QM) meetings.

• Manage the full care planning process and interdisciplinary collaboration.

Qualifications

• Active, unencumbered RN or LPN license in Ohio.

• Nursing degree from an accredited institution.

• 1+ years MDS Coordinator experience in long-term care.

• RAC-CT certification preferred.

• Current CPR certification.

Ideal Skills

• Expertise in Case-Mix, Medicare PPS, and Medicaid reimbursement.

• Strong grasp of federal/state long-term care regulations.

• Excellent communication, critical thinking, and prioritization skills.
