Job Description:
- Track and run assessment reports from UAS.
- Upload data exchange files to various internal and external systems.
- Coordinate and assign new member enrollments across the department.
- Run and compile CM compliance reports.
- Run and compile CM department reports.
- Manage efaxes, hospital notifications and RHIO alerts.
- Manage department consents and their expirations, including RHIO and CDPAS DOH 4359 Forms.
- Provides administration and department support for care management team.
- Assists in completing activities related to different projects/initiatives in support of departmental and organizational goals.
- Maintains and supports documentation, logs and data entry to monitor activity and/or outcomes in support of departmental goals as needed.
- Update CM manual and other documents as needed.
Specific Knowledge, Skills, and Abilities:
- Proficient in the use of office equipment and the computer: Microsoft Word and Excel.
- Outstanding organizational and interpersonal skills.
- Familiarity with MS Office and various business software.
