Job Description

QUALIFICATIONS:

  • LPN, NYS Certification.
  • One year related experience desirable.
  • Ability to transfer individuals with physical disabilities.
  • Possession of a valid New York State driver’s license.

ESSENTIAL RESPONSIBILITIES:

  • Monitor the health needs of the residents.
  • Arrange medical appointments as requested by supervisor and accompany residents to medical appointments or to hospitals for emergency medical treatment.  Ensure proper medical follow-up.
  • Provide care for minor illnesses, emergencies, or injuries of residents.
  • Administer medications, and supervise direct care staff in the proper techniques of handling medications.
  • Keep all medical and nursing records current, including daily, monthly and quarterly reporting, as required by the R.N.
  • Monitor procedures for maintaining records on personal hygiene of residents, including but not limited to; baths, shampoos, toothbrushing, bowel movements, menstruation cycles, and all other required documentation as requested by the supervisor.
  • Monitor the individual medical programs for each resident and maintain nursing care and health care plans for each resident for nursing services, as requested by the R.N.
  • Provide training for residents in the habits of personal hygiene, family life, sex education, and other areas as needed.
  • Control of communicable diseases and infections by identification and assessment, implementation of appropriate protective and preventative measures, and required reporting.
  • Provide in-service training to staff members, such as: detection of signs of illness, basic health skill needs of residents, safety and sanitation, and self-care.
  • Check residents' equipment for repair needs and cleanliness.
  • Assist direct care staff in providing residents with personal and nursing care, including but not limited to: ROM, feeding, treatments, and personal hygiene activities.
  • Ensure adherence to all Agency and Residential Program policies and procedures.
  • Maintain current CPR certification and any other certifications as needed.
  • Perform any other related duties as directed by the R.N. and/or House Manager or designee.
