We have an immediate opportunity for Registered Nurses or Licensed Vocational Nurses with recent Private Duty Nursing Home Health experience. Full Time and Part time positions available.  

CASE DETAILS:

17 YRS OLD MALE GTUBE, CPAP

MONDAYS-THURSDAYS 7A-7P DAYS (SCHOOL NURSE)

MONDAYS, THURSDAYS, FRIDAYS, SATURDAYS, SUNDAYS 7P-3A NIGHTS

 

Duties

• Provides direct, hands-on care to assigned clients.

• Observes and assesses the client’s condition and care needs appropriately and reports changes in condition to the supervisor and/or physician, as appropriate.

• Administers prescribed medications, treatments, and therapies.

• Medication Monitoring

• Takes appropriate nursing action based on current assessments and achieves expected outcomes.

• Educates and provides ongoing support and education to families and caregivers.

 

Experience

• Current, unencumbered Texas license to practice as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN)

• Current CPR certification

• Reliable transportation

• Meet physical demands of lifting, transfers, kneeling and standing.

• Pediatrics experience is recommended, but not necessary. In-Office and On-Site training provided

Transportation: Reliable transportation, valid and current auto liability insurance (if driving patients).

Environmental and Working Conditions: Work in patient's home in various conditions; possible exposure to blood and bodily

fluids and infectious diseases; ability to work flexible schedule; ability to travel local; some exposure to unpleasant weather; PRN emergency call.

Physical and Mental Effort: Prolonged standing and walking required, with ability to lift and move patients.

Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resources acquisition; meet patient/family individualized psycho social needs.

Requires hand/eye coordination and manual dexterity.

Standard Evaluation Assess home health patients to identify the physical, psycho-social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.

Implement/ develop/ document the plan of care to ensure quality and continuity of care.

Provide care utilizing infection control measures that protect both the staff and the patient. (OSHA)Nursing reflects compliance with State, Federal, & Local regulations.

Comply with all First Step Nursing Services policies and procedures.

Provide effective communication to patient/family, team members, and other health care

professionals as evidence by clinical notes, case conferences, communication notes, and evaluations.

Notify supervising nurse of changes in the client's condition immediately.

Provide health education to client and or caregiver.

Documents interim changes of plan of care in timely fashion.
