Requirements:

  • Active RN license (Connecticut)

  • Current CPR certification

  • Driver’s license & reliable transportation

  • Strong clinical, communication, organizational & time management skills

  • Tech-savvy and comfortable with electronic health records

  • Home health experience preferred

  • Familiarity with Medicaid, Medicare, Sandata EVV system a plus

  • Bilingual (Spanish) candidates encouraged to apply – salary incentive available

  • Must be able to perform physical tasks (lifting, transferring, prolonged standing, etc.)


Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans

  • Reevaluate patient needs and update care plans accordingly

  • Provide hands-on nursing care, administer medications, and educate patients/families

  • Coordinate care and act as Case Manager for your assigned caseload

  • Assist with discharge planning and health instruction
