Summary: Care Coordinator - ICC (Dickson and Surrounding Counties): Provides coordination and consultation for the physical and mental health care of clients participating in grants and/or Medicaid Services. Valid driver’s license, proof of automobile insurance, and reliable transportation. Training in proper crisis prevention initiatives and universal precautions required. A bachelor’s degree in a Behavioral health field or 5 years of qualifying experience working with individuals with health disabilities is required. The Care Coordinator is focused on the whole health of our clients with appropriate referrals and collaboration with team members and specialists for conditions requiring monitoring or medical intervention.
