Summary: Care Coordinator - ICC (Dickson and Surrounding Counties): Provides coordination and consultation for the physical and mental health care of clients participating in grants and/or Medicaid Services. Valid driver’s license, proof of automobile insurance, and reliable transportation. Training in proper crisis prevention initiatives and universal precautions required. A bachelor’s degree in a Behavioral health field or 5 years of qualifying experience working with individuals with health disabilities is required. The Care Coordinator is focused on the whole health of our clients with appropriate referrals and collaboration with team members and specialists for conditions requiring monitoring or medical intervention.

Ability to gather information to make informed recommendations and referrals to appropriate resources.  
Ability to help clients identify their strengths and needs. 
Ability to assist with the development, implementation and monitoring of individualized treatment goals. 
Ability to manage a complex caseload of high-need, chronically mentally ill clients. 
Ability to work in an Electronic Client Record. 
Ability to travel, access to reliable transportation and current auto insurance required. 
Access to reliable home internet service required.
