Responsibilities

  • Implementation of physical therapy treatment plans of care.
  • Coordination of care with entire care team
  • Educating patients and family members
  • Preparing timely, complete and accurate clinical notes
  • Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Qualifications

  • Physical Therapist/DPT licensed in the State of Texas
  • Skilled Nursing and Long Term Care
  • Experience with EMR (PCC and NetHealth) is preferred 
  • Cares for people and will go above and beyond to provide outstanding patient care
  • Scheduling flexibility that allows you to do your best work while enjoying great quality of life
  • Demonstrates a dedication for outstanding delivery of patient care and teaching
