Position Title: Family Support Partner and Community Connector
Reports to: State coordinator for Horatio, State program director, and appointed Horatio school staff
Collaborates: Works closely with the current family support partner from Horatio
Job Overview
The Prevention Pilot Project FSP will provide intensive in-home intervention to support families and children/youth in the Partners’ Prevention Pilot Program. The purpose of this pilot initiative is to assist families/children/youth at risk of losing their current placement in their home or educational setting (e.g. daycare, early education center, classroom, etc.) The FSP will walk alongside the family and children to implement an intensive intervention plan. The FSP will help with access to appropriate community services, develop pilot family support plans, and monitor progress and ongoing needs.
A key component of the program is building collaborative teams. The FSP will collaborate with community partners, the pilot and state level programs, to promote and implement intensive intervention plans. As a community connector the FSP will organize and facilitate community activities and events to promote engagement for the children/youth in the program.
FSPs must be committed to providing high quality support that is focused on the families’ needs and priorities in a manner that is honest and respectful.
Candidates should have the following qualifications and experience:
Education:
Minimum of a bachelor’s degree in social services, education, psychology or related field.
Years of experience can be substituted for educational requirements.
Experience:
Working with children/youth/families experiencing difficulty.
Additional Requirements:
Must have a heart for supporting children/youth and families in their community to enjoy a better quality of life.
Must have positive people skills and be able to form effective working relationships with a wide range of people.
Must have the ability to listen and solve problems.
Must have communication skills that enable effective, courteous interaction with families and team members.
Must protect child and family confidentiality, following appropriate procedures for sharing information.
Must have the ability to create, organize, and collaborate with local partners to develop community youth events.
Must be willing to develop empowering opportunities for families and youth that promote stability, and engagement between families, schools and community partners across the county. Horatio school district is the primary partner, but we also work with early childhood programs and other school districts when needed.
Must pass required background checks and drug screen.
Must be able to lift to 30 pounds and climb stairs.
Must have a valid driver's license and reliable transportation, travel is required.
Must be available evenings, weekends, and be on call when needed. Most of the work is conducted during times when children and families are available.
Must be willing to complete required training.
Must be able to travel locally and within the state. No out-of-state travel is required.
Must be able to be a self-starter and manage appointments, meetings, and events from a home office.
Must be able to communicate and share work schedules, program activities, and child/family status with pilot team, school officials, and community partners as needed while protecting confidentiality.
Salary Range: Salary is based upon education and experience and determined by state and school leadership.
