This role offers the opportunity to provide personalized, one-on-one care in patients’ homes, helping individuals regain independence and improve quality of life through skilled, patient-centered therapy.

Department: Home Health
Employment Type: Full-Time or Part-Time
Schedule: Monday–Friday, with flexibility based on FTE and patient care needs

 

Essential Duties and Responsibilities

  • Conduct comprehensive in-home occupational therapy evaluations and develop individualized treatment plans

  • Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve

  • Complete and score OASIS assessments accurately and in compliance with Medicare regulations

  • Maintain knowledge of payer guidelines and Medicare Conditions of Participation

  • Participate in interdisciplinary case conferences and QAPI initiatives

  • Supervise Occupational Therapy Assistants (OTAs) and provide oversight to CNAs when applicable

  • Manage patient care episodes including admissions, reassessments, and discharges

  • Document services and communications accurately using point-of-care electronic systems

  • Coordinate daily schedules to support continuity of care and efficient travel

  • Maintain active Montana OT licensure, required continuing education, and ethical professional standards

  • Participate in agency training and professional development activities

Requirements

REQUIREMENTS

  • Bachelor’s degree or higher from an accredited Occupational Therapy program

  • Current Occupational Therapist license in the State of Montana (or ability to obtain)

  • Valid driver’s license, reliable transportation, and appropriate auto insurance

  • Access to high-speed internet

  • Two or more years of occupational therapy experience preferred

  • Home health or clinical rehabilitation experience a plus
