Job Description

This position provides high quality, client-centered intensive supportive services to clients in permanent supportive housing through Housing for Health. The clients are DHS patients who meet/met the HUD definition of Chronically Homeless and were/are high utilizers of DHS services.

Case management duties include outreach and engagement; intake and assessment; service planning; housing and rental assistance; ongoing monitoring and follow-up; assistance with benefits establishment, transportation, and legal issues; crisis management; eviction prevention; client education; housing location services; and coordination and collaboration with HFH partners. This program incorporates harm reduction strategies to support participants in permanently retaining their rental housing.

Duties and Responsibilities

Outreach & Engagement

  • Process and accept referrals from DHS and HFH partners
  • Establish rapport and determine client eligibility
  • Conduct face-to-face screenings within 2–5 days of referral
  • Obtain necessary consents and authorizations
  • Refer ineligible referrals to appropriate services

Intake & Assessment

  • Conduct intake and enrollment
  • Assist clients with collecting eligibility documentation

Temporary Housing, Rental, and Move-In Assistance

  • Assist with temporary housing referrals until permanent placement
  • Coordinate lease agreements and client move-ins
  • Conduct psychosocial assessments and update service plans every three months
  • Maintain regular contact based on client needs and acuity level
  • Link clients to physical, mental health, and substance use services
  • Support medication compliance, income/benefits access, employment, life skills, and budgeting
  • Provide 24/7 on-call crisis support and educate on appropriate use of emergency services
  • Educate clients on tenant rights, responsibilities, and lease compliance
  • Provide eviction prevention counseling and document interventions
  • Coordinate transitions for clients with changing housing or care needs

Client Satisfaction

  • Conduct one-on-one check-ins and meetings to assess satisfaction
  • Administer annual client surveys and adjust services based on feedback

Collaboration

  • Attend meetings with DHS and HFH partners
  • Conduct case conferences with supervisors and team
  • Maintain partnerships with community service providers

Qualifications and Requirements

  • Bachelor’s degree in Social Work or related field and 1 year of relevant experience, or 2 years of direct mental health/intensive case management experience
  • Experience with homelessness, mental health, chronic health, and substance use issues
  • Strong documentation and case management skills
  • Familiarity with crisis intervention, eviction prevention, housing rights, and benefits systems
  • Proficiency in Microsoft Word and Excel
  • Valid driver’s license and active auto insurance
  • Ability to drive agency vehicles and meet insurance requirements
  • Maintain confidentiality in compliance with HIPAA
  • Must attend all required meetings and trainings
  • Ability to manage a caseload of 20 clients

Physical Requirements

  • Driving is required as needed to perform job duties
  • Prolonged sitting, standing, and frequent bending
  • Occasional lifting up to 25 lbs
  • Frequent use of computer and phone
