Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties.  It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network.  The network works together to create opportunities for older Ohioan's to receive needed home and community services and supports; and to age successfully in their own homes and communities.

Position Overview: The role of Community Resource Consultant is to initiate the process of evaluating the individual/consumer needs and providing information regarding available options.  



 



Job Duties: 




  • Assessment availability emailed to direct supervisor and (scheduling supervisor) at least 5 business days in advance.  

  • Email Clinical Manager and Supervisor the number of assessments completed for that pay period by 3pm of the last Friday of that pay period.  Include consumer name/number and date completed. 

  • Adherence to all agency and departmental policies and procedures must be maintained. 

  • Emails should be monitored and responded to on working days or at least weekly.

  • Monthly 1:1 meeting with supervisor as coordinated between the two parties.

  • Complete face to face assessments (all types) in the community with Consumer (and caregiver/contact) to identify unmet needs, determine level of care, and identify health and safety risks or concerns. 

  • Educate consumers, caregivers and stakeholders regarding eligibility of WRAAA administered programs.

  • Communicate and collaborate with caregivers, healthcare and service providers, and any other formal and informal systems to accurately determine and meet the individual’s needs. 

  • Provide options and referrals to meet individual’s needs if not pursuing Medicaid Waiver. 

  • Document in appropriate software system according to policies and quality compliance standards. 

  • Work cooperatively and constructively with internal WRAAA staff, and with formal and informal systems.  

  • Advocate on client’s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization. 

  • Collaborate and participate in department and agency initiatives for the purpose of improving quality, efficiency, and positive stakeholder interactions. 

  • Complete assessments during core business hours and occasionally early evening or weekend hours as needed to meet the needs of the community. 

  • Adhere to minimum capacity requirements. It is the sole responsibility of the CRC to meet productivity standards. 

  • Participate in training, meetings, best practice committees and LEAN projects. 

  • May be asked to provide assistance and back-up to other units within the department and/or other duties as assigned.
