Position Summary: Responsible for initiating home care services for newly referred clients. Additionally, the Start of Care Specialist will match appropriate home care aides with client�s to promote independent living at home.

Hours: Full Time Monday through Friday 8 am - 5 pm

Location: 2333-2341 W. 95th Street Bldg. C, Suite 2 Chicago IL

Essential Duties:

  • Initiates Home Care Services for new clients in accordance with established payor timelines
  • Facilitates communication between new clients, home care aides, case managers and the branch
  • Completes and coordinates client/employee schedules in accordance with the client�s plan of care while remaining within authorized service hours
  • Communicates changes in service needs to referral agency staff and branch management
  • Schedules preferred workers/family caregivers for pre-employment training
  • Resolves consumer complaints within the scope of authority
  • Notifies branch management of significant events related to service delivery
  • Notifies Service Coordinators of new clients received within assigned service area
  • Performs other duties as assigned

Position Requirements & Competencies:

  • High school diploma or equivalent
  • Data Entry
  • Customer Service
  • Case Management
  • Computer proficiency - MS Office
  • Occasional travel between facilities may be required
  • Must have reliable transportation, valid driver license and state required insurance
