Description:
Manage nursing care for an assigned caseload, support field nurses, and ensure timely, accurate, and compliant documentation. Collaborate with patients, families, physicians, insurance case managers, and our interdisciplinary team to facilitate appropriate, high-quality care.

Job Duties:
  •  Oversee and coordinate nursing care for assigned patients
  •  Guide and support field nurses and serve as a clinical resource
  •  Review clinical documentation for accuracy, completeness, and compliance
  •  Communicate with patients, families, physicians, insurance case managers, and internal teams to support safe and timely care
  •  Participate in quality improvement efforts and adhere to agency policies and standards

Requirements:
  •  Valid New York RN license in good standing
  •  Associate’s degree required; BSN preferred
  •  Minimum of 2 years of nursing experience
  •  At least 1 year of home care experience required
  •  Strong communication and computer skills

Compensation & Benefits:
  •  Salary: $85,000 annually
  •  Medical: Individual medical coverage with a minor employee contribution
  •  Paid Time Off: 4 weeks of PTO annually

Schedule:
  •  Full-time, 5 days per week, 35 to 40 hours per week
