A DAY IN THE LIFE OF A NURSE PRACTICIONER
As an NP, in collaboration with the Hospice Medical Director, and referring physicians, this position is responsible for carrying out Face-to-Face visits for Hospice patients for the VNAHSR. Responsibilities include but are not limited to the following:
- Gather clinical information to support the diagnosis and prognosis and provide these findings from the face to face encounter to the physician who is considering recertifying the patient.
- Attest to a face to face encounter being made and includes in the attestation: The beneficiary's name, the date of the encounter, the certification period dates, and that the clinical findings of that encounter have been provided to the certifying physician for use in determining continued eligibility for hospice care.
- Provide care to patient when a need is identified during a face to face encounter. Document the clinical finding and care provided in the patient's medical record.
- Complete a face-to-face encounter for a skilled assessment of terminally ill patients.
- Attest to a face to face encounter being made and include in the attestation.
QUALIFICATIONS FOR A NURSE PRACTICIONER
- Graduation from an accredited school of nursing. Master's degree and advanced practice nursing degree as a nurse practitioner required.
- 2+ years of nursing experience is preferred
- Valid Vermont nursing license required. Valid Vermont State (adult) nurse practitioner license required.
