Job Description
The Social Work Clerk provides administrative and clerical support to the Social Services
Department in a nursing home setting. This position assists the Social Worker and
interdisciplinary care team in maintaining accurate resident records, coordinating communication
with residents and families, and supporting daily departmental operations. The Social Work
Clerk plays a vital role in promoting quality care and ensuring that residents’ psychosocial and
administrative needs are met efficiently and compassionately.
Job Duties
-Assist the Social Worker in completing, filing, and maintaining psychosocial assessments,
progress notes, and care plan documentation.
-Prepare and update resident charts and records in accordance with facility policies and
regulatory standards.
- Organize and maintain departmental files, correspondence, and reports as delegated by the
Social Services Director.
-Review and Issue Notice of Medicare Noncoverage and ABNs timely and efficiently and
document accordingly
-Assist with meeting new residents and their family/representatives upon admission and gathering
information
-Provide clerical support during admissions, transfers, and discharges, including collecting
required documentation and updating records
-Assist with referrals and follow-up communications with community agencies under the
direction of the Social Worker
-Gather and input information to support timely and accurate completion of MDS assessments under the direction of the Social Worker
