Hospice Aide Coordinator  


What You Must Have:

  • High School Diploma Preferred
  • If an aide, has successfully completed one of the following:
    1. Training program/competency eval
      as specified in §418.76(b)(c)of the Medicare regulations
    2. Competency eval program that meets §418.76(c)
    3. Nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry
    4. State Licensure program that meets requirements of §418.76(b)(c)
  • Licensure: CNA preferred
  • At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
  • One year experience as an aide scheduler preferred.
  • CHPNA certification a plus
  • EMR experience a plus
  • Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

  • Utilize the EMR software system to schedule clinical staff patient visits
  • Schedule patient visits according to care plans and staff availability
  • Confer with Patient Care Manager and/or Clinical Supervisor on problems and complaints timely
  • Verify documentation of information such as completion of visits, frequency & duration of visits
  • Oversight and tracking of staff in regard to punctuality, assignments and reliability
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
