Provides individual and group education to those with Diabetes and other nutritional needs. Addresses educational needs of other health care professionals who do not specialize in diabetes or nutrition, and the public. Collaborate with other members of the health care team to establish treatment plans.
Provides Medical Nutrition Therapy and Diabetes education for individuals in the outpatient setting. Education is offered at varied approved education sites.
Documents patient care to meet requirements of accreditation and reimbursement standards.
Designs a plan of care that provides for optimal patient outcomes.
Collaborates with physician, homecare and other specialties in the healthcare community to implement and evaluate the plan of care.
Participates with other members of the education team to plan, implement and evaluate American Diabetes Association (ADA) accredited Diabetes Management Education series.
Serves as a resource to other Beebe Healthcare professionals within area of expertise. Participates in community education events. Participates actively in Beebe Healthcare committees and meetings as deemed appropriate by supervisor.
Completes required reports and maintains statistics.
Participates in department process improvement initiatives, continually striving for more efficient quality service.
Maintains membership in and participates in professional organizations. Performs other duties within the scope of practice as designated by supervisor.
Participates in team efforts to promote education services and increase number of patients served.
Graduate of an accredited bachelor's Program in Nutrition or related field.
Licensed as a Dietician / Nutritionist in the State of Delaware. Successful passage of American Dietetic Association Registration Exam and current Registration with the Commission on Dietetic Registration (CDR) as a Registered Dietician.
Certified Diabetes Educator (CDE) within 2 years of hire.
