Job Description

We are seeking a Home Care (Intake Program Assistant) who will be responsible for providing administrative support to the Home Care Department.

What you'll do:

  • Provide telephone coverage from Home Care staff as required
  • Communicate with the program management regarding information related to file set up, issues with completed paperwork, and quality of referrals being submitted
  • Review and correct data for new consumers within record system
  • Provide accurate reports as needed
  • Assist staff in sending and submitting waiver paperwork for consumers
  • Other duties as assigned

  • Ability to deal effectively with statistical and financial data
  • Knowledge of general office procedures and equipment
  • Minimum one year of relevant experience
  • Associate's degree preferred
  • Excellent written and verbal skills.
  • Excellent computer skills.
  • Proficient with Microsoft Office Suite or related software.
