General Information:

Compensation We Offer  

  • The initial compensation for this position ranges from $73,469.00 - $90,358.00 per year; depending on experience, location, and internal equity considerations.  
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD. and select Peer Certifications.
  • 5% Bilingual Differential for qualified positions

Overview:

Works within the values and clinical philosophy of the agency. Under the general supervision supports Program Managers-Managed Care and Program Supervisors- Managed Care by ensuring quality comprehensive care management assessments (including risk, social determinants of health) and plans in managed care programs consistent with the requirements of CalAIM.



Responsibilities:
  • Apply advanced skills to gather assessment information in development of the care plan and applies information and resources to ensure quality care.
  • Understands and applies multi-modal approaches and perspectives to whole person care.
  • Actively participates in the identification of team opportunities for improvement or identification of deficiencies and in the development of strategies to address or bridge gaps.
  • Participates in Quality Improvement (CQI) activities and project implementation.
  • Performs other related responsibilities, as assigned, to support specific department/business needs.


Qualifications:
  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • Masters degree in Social Work or a closely related field (e.g., Marriage, Child and Family Counseling or Psychology) required.
  • Three (3) years of supervised clinical experience.
  • Licensed according to California state law (Welfare and Institutions Code section 5600.2)
  • Minimum of 2 years of clinical experience with complex care
  • Experience with care management plans.
  • Knowledge of Social Determinants of Health and Crisis management experience preferred.
  • Strong clinical and crisis management skills.
