Case Manager [Full Time / Reports to Administrator]

The Dream Center Indianapolis Inc. is seeking a passionate and dedicated full-time Case Manager to join our team. At The Dream Center, our mission is to disrupt and reverse the cycle of poverty in Indianapolis. We achieve this through our comprehensive Pathways System, focusing on empowering single mothers through work development, financial coaching, and mindset training. Our ultimate goal is to significantly increase our clients' annual income and guide them toward financial independence, reducing their reliance on government and community assistance programs such as Medicaid, SNAP, and food pantries.  

As a Case Manager at The Dream Center, you will play a vital role in directly supporting our clients on their journey to self-sufficiency. You will be responsible for assessing their individual needs, developing tailored plans of care, proactively addressing potential challenges, and meticulously tracking their progress and outcomes. This position requires a highly organized, empathetic, and results-oriented individual who is committed to empowering others.

Responsibilities:

  • Conduct thorough assessments of new clients to identify individual barriers to success and determine appropriate levels of support.
  • Develop individualized plans of care aligned with each phase of the Pathways System, ensuring clear milestones and measurable objectives for each client.
  • Proactively monitor client progress and identify any indicators of potential setbacks or delays in achieving milestones.
  • Implement assigned protocols and interventions in a timely and effective manner to support clients in overcoming challenges and staying on track for success.
  • Maintain accurate and up-to-date records of all client interactions, progress, and outcomes within the client tracking system.
  • Provide clients with the necessary tools, resources, and support to foster their satisfaction and engagement in the program.
  • Track and analyze client outcomes and results to evaluate the effectiveness of interventions and contribute to continuous program improvement.
