RAC Responsibilities:

  • Conduct ongoing assessments of residents' physical, emotional, and social needs to develop and implement comprehensive care plans

  • Collaborate with healthcare teams to ensure residents receive seamless care and services

  • Monitor and report changes in residents' conditions to ensure timely interventions

  • Maintain accurate and up-to-date records of assessments, care plans, and progress notes

  • Develop and implement strategies to prevent and manage resident risk factors

  • Participate in quality improvement initiatives to enhance resident outcomes

  • Provide education and support to residents, families, and caregivers

RAC Requirements:

  • Bachelor's degree in Nursing, Social Work, or related field

  • Minimum 1 year of experience in healthcare, preferably in long-term care or rehabilitation setting

  • Strong assessment and communication skills

  • Ability to work effectively in a fast-paced environment

  • Strong organizational and time management skills
