ORGANIZATION DESCRIPTION 

 

Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) aims to promote the social and economic empowerment of Chinese American, immigrant, and low-income communities. Today, CPC is the nation's largest Asian American social services agency and offers more than 50 programs at over 35 sites throughout New York City, including early childhood education, school-age care, youth services, workforce development, community services, and senior services. CPC strives to be the premier social service and leadership development organization. In 2017, CPC launched Advancing Our CommUNITY, its organization-wide strategy to expand services to address persistent needs and emerging trends and to improve leadership skills among staff and community members. In 2022, our core programs and emergency services reached 200,000 people living in all five boroughs and all 51 city council districts. PROGRAM SUMMARY 

 

Asian Family Services (AFS) provides child welfare preventive services to help keep children and their families together and avoid the placement of children in foster care. AFS addresses the multi-level needs of Asian-American families, especially recent Chinese immigrants by providing support, advocacy, and referral to services and case management to Asian American families throughout the five boroughs. Our professional staff consists of bi- and tri-lingual and bi-cultural caseworkers 

 

JOB SUMMARY 

 

Reporting to the Director of Asian Family Services, the Family Team Conference Coordinator will be facilitating collaboration between families, the agency, and the Administration for Children’s Services (ACS). The Family Team Conference Coordinator is responsible for coordinating and facilitating Family Team Conferences, ensuring effective communication between the agency and ACS, and supporting timely documentation and follow-up. The Coordinator may also carry a caseload as needed, assessing safety and risk while helping families navigate supportive services.  

 

ESSENTIAL RESPONSIBILITIES 

 

  • Coordinate meetings and facilitate Family Team Conferences in-office and off-site.
  • Act as the liaison between the agency and ACS when setting up the conferences.
  • Document all outcomes from the Family Team Conferences and ensure that they are executed in a timely manner.
  • Take on cases on an as needed basis.
  • Assess and manage cases, evaluating for safety and risks.
  • Compile monthly statistics and other reports as needed by the program and funding source.
  • Other duties as assigned by the Supervisor and Program Director.
